Real Panchakarma requires 14-28 days minimum with medical supervision. Legitimate centers: Barberyn Reef (Beruwala, family-run since 1968, medically staffed, prescribed by Vaidya), Ulpotha (very traditional, seasonal, deep-rural, no wifi, USD 300-400/day), Santani (Kandy, blends wellness with luxury). All require in-take medical consultation before treatments.
Genuine Panchakarma includes: initial doctor consult, Purvakarma prep (oil massage, steam), then the 5 main cleansing therapies (Vamana emetic, Virechana purgation, Basti enema, Nasya nasal, Raktamokshana blood cleansing — most tourists do 2-3 of these based on constitution). Prescribed personalized herbs. Not a spa vacation.
