Both mobile but slow. Chauffeur car ($80/day) covers driving. But do I also need porters, guides, medical support...?

Chauffeur is enough baseline. Add-ons based on need: (1) Porter service is free at most hotels 3-star+ (staff carries bags, no need to arrange). (2) Guide only for cultural sites where they'll enjoy context (Sigiriya, Anuradhapura). (3) Pre-arrange doctor-on-call service if any medical concerns - Ceylon Medical Concierge does this for $200-400/day if needed. For 70s couple with basic mobility: chauffeur + boutique hotels + slow pace. That's it.

^ agree. Also: get travel insurance with GOOD evacuation coverage (Medjet, IMG Global). Not because they'll definitely need it, but for peace of mind. Colombo private hospitals are excellent for anything short of major surgery, but international evacuation is where you want coverage.
