On the train: move to the doorway for fresh air (safer standing than moving cabins). Ask conductor for the next major stop. Get off at any station with a town nearby (Peradeniya, Kadugannawa, Nawalapitiya on the Kandy-Ella route). Every station town has clinics or private hospitals within 500m.

Prepared travelers carry: ORS sachets, paracetamol, ginger tablets for nausea, small plastic bag. In an emergency, message your accommodation via WhatsApp — they arrange PickMe from station to your hotel or hospital. Sri Lankan railway staff are generally kind and helpful in emergencies.
