How much English is spoken in rural areas? My Sinhala = Google Translate.

More than you think. Sri Lanka has high English literacy (English is a mandatory school subject). Even in rural areas, someone will speak basic English - a school teacher, a hotel staff, a shop owner's daughter. Full conversations may be a challenge in remote villages. Google Translate camera translate handles menus and signs. For key phrases (directions, numbers), learn 5-10 Sinhala/Tamil words. Locals appreciate the effort and warm up fast. You won't be stranded.
^ this. Also: hand gestures + facial expressions + patience get you very far. Sri Lankans are patient with foreigners trying to communicate. Not dismissive like some countries. Point at menu, smile, say 'thank you' in Sinhala ('stuti'), and you're doing better than most tourists.
