The venom of Hypnale hypnale contains potent phospholipase A2 enzymes, procoagulant factors, and metalloproteinases. While local pain and hemorrhagic blisters appear rapidly, the critical systemic threat is venom-induced consumptive coagulopathy (VICC) paired with microthrombi formation inside renal glomeruli. This induces acute tubular necrosis and renal cortical ischemia. Patients often seem stable on day one, only to develop oliguria, dark urine, and soaring serum creatinine on days three to five. Regular 12-hour renal function panels and early hemodialysis are mandatory.
