Legit Ayurveda doctor vs another spa - Google is useless.

Practical verification: (1) Ask to see the doctor's SLMC (Sri Lanka Medical Council) registration number. Real Ayurvedic doctors are registered. (2) Consultation is 30-60 minutes and includes pulse diagnosis (nadi pariksha), tongue examination, dosha assessment. Spa versions are 5-10 min or skipped. (3) Prescribed treatment plan includes dietary changes, not just massages. (4) Herbal medicines with local plant names, not brand names. (5) Repeat visit expected (they follow up on your treatment progress). Legit centers: Barberyn Reef, Siddhalepa Ayurveda Health Resort, Ulpotha, Santani. Spa versions: any beach resort with 'Ayurveda' in marketing but no SLMC doctor.
^ this. Also: Bandaranaike Ayurvedic Research Institute in Colombo maintains a list of SLMC-registered Ayurvedic doctors. Cross-reference any center's claimed doctor with this list. Real ones show up.
