YES fully disclose. Undisclosed pre-existing conditions void coverage entirely if claim relates. Most policies offer 'stability period' clauses: if condition stable 60-180 days pre-departure, covered. If flare/change occurred recently, coverage may exclude that condition.

For chronic conditions (diabetes, heart disease, cancer history, mental health): specialized medical travel insurance (World Nomads Explorer, Allianz Global Assistance) offers riders. Cost may be 30-50% higher but coverage worthwhile. Disclose in writing during purchase.
