The available Indian polyvalent antivenom targets the "Big Four" (Russell's Viper, Common Krait, Cobra, Saw-scaled Viper), but its efficacy against Sri Lankan Russell's viper venom is imperfect due to geographic venom variations, causing allergic reactions in up to 50% of recipients. Crucially, Indian polyvalent antivenom has zero neutralizing efficacy against the Hump-nosed Pit Viper (Hypnale hypnale), which causes the highest bite frequency in the country. Hypnale bites require supportive care and close monitoring for acute kidney injury rather than unindicated antivenom.
