Water fasting >72 hours SHOULD be medically supervised. Legitimate centers: Barberyn (medically supervised intermittent and juice fasts as part of Panchakarma), Santani (guided cleanse programs). Pure water fasts 5+ days require doctor screening for cardiovascular safety.

For serious water fasting, be honest about medical history, take blood pressure/glucose baseline, and stay hydrated with electrolytes. NOT recommended for anyone on medication for hypertension, diabetes, or eating disorder history. Ayurveda's approach is gentler graduated cleanses, not extreme water fasts.
