A bartender at a southern beach rave offered us a complimentary traditional Ayurvedic herbal shot to keep us dancing. Later people said these are sometimes spiked with illicit substances. Is this true.

Yes, be incredibly careful. While some places just offer harmless ginger or coriander shots, sketchy pop-up raves sometimes lace these "free herbal drinks" with cheap local stimulants or unregulated substances to keep you buying alcohol at the bar. Never accept unsealed or free shots at underground parties.

If it is a reputable club (like Dots or The Doctors House) it is probably just ginger and turmeric. If it is a random DJ tent on the sand, refuse it.

Stick to bottled beer that they open in front of you if you are at an unregulated beach rave.

A real Ayurvedic doctor would never prescribe medicine to be taken while you are heavily drinking alcohol anyway!
