Does the train rock violently in the mountains. Should I take medication beforehand.

The train ride from Kandy to Ella winds through steep mountain passes, meaning the train constantly sways, rocks violently side to side, and jolts forward as it climbs. If you are highly prone to severe motion sickness, sitting in an enclosed, air conditioned first class carriage facing backwards can definitely trigger nausea. Taking a standard motion sickness pill an hour before boarding is a very smart precaution to ensure you enjoy the beautiful scenery.
