The fish spas on the Madu River are a very popular tourist stop. The tanks are usually open to the river water, which provides a natural flow. While it is generally safe for humans, ensure you do not have any open wounds on your feet. As for the fish, they are well fed and accustomed to the interaction. It is more of a fun, ticklish experience than a clinical treatment. Most boatmen include it as a standard ten minute stop during the lagoon tour.
