The food is very safe in terms of hygiene because it is cooked fresh. However, the health concern is the SPICE level. Authentic Jaffna crab curry uses heavily roasted fiery chili. If you have a weak stomach for spice, ask for "tourist mild." For the palm toddy, only drink it before 10 AM. Afternoon toddy ferments in the heat and will definitely upset your stomach.

I had the spicy crab curry and was sweating buckets, but it was the best meal of my life.

Drink it out of the folded palmyra leaves if they offer it. Great cultural experience.

Very mildly. It tastes more like sweet, slightly fizzy coconut water in the early morning.
