Yes the doors stay open throughout the journey and locals frequently sit on the steps to enjoy the breeze. However tourists die or suffer serious injuries every single year by leaning out too far and being struck by trackside poles, signs, or oncoming trains. Always keep your head and torso fully inside the door frame. Use one hand to firmly grip the inner handle at all times.
Be extra cautious in the many tunnels along the route especially between Demodara and Ella. The clearance inside tunnels is extremely tight and several tourists have been seriously hurt. Always pull yourself fully inside before the train enters any tunnel. Listen for the warning whistles from the driver.

People outside stations offer to "help" buy tickets but charge large markups. They may buy cheap third-class tickets and sell as more expensive classes.
