If you visit a highly reputable officially registered Ayurvedic clinic they will automatically assign you a female therapist without you even needing to ask. If a tiny random beach spa tries to aggressively assign a male masseuse to a female tourist it is a massive red flag indicating they are completely unprofessional and you should walk away instantly.
