The private hospitals in Colombo (like Nawaloka, Asiri, or Lanka Hospitals) are world-class. English is spoken perfectly, the facilities are modern, and doctors are highly trained (many in the UK or US). An emergency room visit with X-rays will cost a fraction of what it does in Western countries. Travel insurance usually covers it easily.

Outside of Colombo, you will rely on government hospitals. They provide free healthcare to everyone (even tourists in emergencies), but they are crowded and lack luxury amenities. However, the doctors are still excellent.
