Is the dress code as strict as the temples in Jaffna? Do men have to remove their shirts?

No, Koneswaram is slightly more relaxed than Nallur Kovil in Jaffna. Men do NOT have to remove their shirts. However, you must still cover your knees and shoulders (both men and women), and you must leave your shoes at the stalls before walking up the final path.

The walk from the shoe drop-off to the temple is on black asphalt and gets blazing hot by 11 AM. Bring a pair of thick socks to wear while walking!

You can buy small offerings of fruit and flowers from the stalls on the way up to give to the priests. It is a lovely experience.
